The result is not usually one big visible failure, but a series of gaps: unclear priorities, weak alignment between business and IT, duplicated analysis, avoidable rework, low adoption, or solutions that work technically but do not fully support the way the organisation needs to operate.

A new ERP, workflow platform, AI solution, or reporting tool may technically work well while still creating confusion, rework, or low adoption if the organisation has not clarified what needs to change operationally.

This is why I focus on aligning Process × Data × Systems together rather than treating technology as an isolated layer.
